Donna Moser BSN RN CHFN has had the great honor of serving the Philadelphia community as a nurse for 37 years. She is passionate about people receiving much-needed health care in their homes and in the community. Throughout her career she was part of Esperanza Health Center providing bilingual health care to the Hispanic community, twenty-five years of home visiting nursing, and the last 8 years serving at Einstein Cardiology providing heart failure education, coordinating the Heart Failure/ ICD/ LVAD support group and promoting social equity in the management and prevention of heart failure.

Her “Addressing Social Inequity in the Treatment and Prevention of Heart Failure: Equal Access in Blood Pressure Management” was presented at the 2022 Annual Meeting of the American Association of Heart Failure Nurses (AAHFN) and her project “Reducing Readmission Utilizing ReDS Technology” was given the Peer Review Scientific Merit Poster Award in the category of Innovative Programs (2020).  Additionally, she is Secretary for the AAHFN Advocacy Committee mobilizing awareness of heart failure patient issues on a national level. She received the Einstein Healthcare Network Jokline Joseph BSN RN Caring Quality Award 2023.

When not advocating for patients, she volunteers at the Living Word Community church, teaching children, and accessing food for neighborhood food pantries and outreach programs.  She also enjoys being a Peer Mentor through Parent2Parent, a Pennsylvania resource program matching individuals with parents and caregivers of those with special needs.  She is the grateful wife (36 years) of C. Daniel Moser PhD, and proud mother of five, grandmother to 3 and friend to all.
